    Job Description
    NOTICE OF OPEN POSITION
     
    TITLE:                Physical Therapist-EI
     
    LOCATION:       Margaret B. Rost School

                           Cincinnati, OH  45248
     
    QUALIFICATIONS:  Licensed Physical Therapist in the State of Ohio, Meets the agency’s driver insurability requirements.
     
    DUTIES:        (Not intended to describe all duties.)
    As part of a multi-disciplinary team, performs professional physical therapy (PT) evaluations and assessments using related PT formal/informal procedures, tests and tools to establish child/family’s eligibility and need for ongoing services.
     
    Develops Individualized Family Service Plan (IFSP) outcomes based on family priorities and PT practices and techniques as part of a trans-disciplinary team; researches and secures adaptive equipment and devices; works cooperatively with other staff, service facilitators, parents, caseworkers, medical personnel, specialists, etc. to achieve child and family outcomes as identified by the IFSP.
     
    Implements evidence-based early intervention PT services through a primary service provider approach; utilizes a coaching interaction style with other staff, parents and caregivers (i.e., records results of assessments and documents therapy given, summarize physical therapy assessment and treatment results, provides in-service training and individual instruction to agency personnel in handling, positioning and adaptive equipment).
     
    Provides PT related services within natural environments such as homes and daycare centers; travels to and gains access to locations in the community to implement plans based on individual’s needs in a safe, efficient and effective manner in accordance with agency procedures.
     
    Communicates family’s priorities, needs and interests; provides coaching support to team members during weekly team meetings and on joint visits; contributes disciplinary expertise related to PT functions in regard to motor and self-care function and necessary augmentative and rehabilitation supports.
     
    Develops and maintains individual records and reports.
     
    Participates in continuing education opportunities within the field of early intervention and physical therapy to ensure current, relevant, evidence-based service delivery; maintains required licenses and/or certificates.
     
    Demonstrates regular and predictable attendance, performs other related duties as required or assigned.
     
    COMPENSATION:
    Annual Salary:  $51,178.40 (based on 35 hours/week)
