Required Forms

1. Applicants must complete the form below
2. The deadline is August 15, 2024
3. Notifications and funds will be sent out during August-September 2024.
4. A midyear report is due December 31, 2024, and a final report is due June 30, 2025.  All funds must be used within the year.  The outline for the report will be included in the acceptance letter. Send reports to michael@HAHCincinnati.org
